How do I report symptoms to my healthcare provider through the ImPACT Passport app?

Click on your Name/Passport ID number saved in ImPACT Passport® app, then click “Record Symptoms”. Answer all the questions and your symptoms will be saved. Your healthcare provider will be able to access this information at any time.  You are also able to send your report via email to your healthcare provider.
